Futures’ discharge planning specialists help patients identify and connect with the specialized aftercare programs or care providers in their home communities that will continue treatment. They may also identify safe recovery housing options for Core patients needing such services. Through careful planning, patients can make a smooth and seamless transition between Futures and the network of ongoing care providers that will help patients maintain recovery and good health.
Case Managers work directly with medical professionals, intensive outpatient programs (IOPs), therapists, physical therapists, and other specialized healthcare providers while patients are in the Core Program at Futures. Case Managers at Futures coordinate this process for patients to ensure that appointments are scheduled prior to discharge from Futures. Patients meet with their case manager weekly to assess and evaluate their needs and how to utilize resources after discharging from the treatment program.
The Futures Alumni Services’ department also plays a vital role in discharge planning. Every Futures’ Core Program patient that completes the program becomes a member of our alumni community. The alumni program is an important resource for patients through its ongoing network of support and empowerment as patients transitioning back to real life. The Futures’ Days of Ascent program welcomes alumni back to the treatment facility for additional support, inspiration, and camaraderie.
