At Futures, case managers are the hub of the wheel. These specialized members of the clinical team make a significant difference in the effectiveness and experience of addiction treatment. Case managers are a patient’s primary point of contact during their time at Futures and coordinate every aspect of the patient’s care and services.

Case management is an often overlooked but vital part of effective, coordinated care. The role of case managers is multifaceted and requires a specialized skill set and experience. Case managers are adept at meeting patient needs and supporting a positive treatment experience while ensuring that they are making positive clinical progress. Case managers regularly assess each patient’s progress and work with their care team counterparts to adjust care plans as needed. They also empower patients by helping them take increasing responsibility for their recovery and ongoing care. Case managers also make treatment stays more practical and less stressful by assisting patients to fulfill important home or workplace obligations without threatening therapeutic progress. A patient’s case manager can arrange the use of phones and computers to correspond with colleagues, family members, and institutions. Case managers excel at ensuring that managing life outside of treatment does not interfere with the critical work of therapy and healing.

Case managers also act as bridges to patient families, professional resources, and ongoing care teams. During treatment, case managers communicate with patient family members to update status and arrange family visits and therapy sessions. They may also interact with patient employers, lawyers, or other specialized service providers as necessary. Case managers are also involved with aftercare and discharge planning. They identify and contact care providers in the patient’s home community and establish a relationship to facilitate a smoother transition into ongoing care after treatment at Futures.
